What the Chain of Custody Form Is and Why It Carries So Much Weight

The Federal Custody and Control Form

In DOT-regulated drug testing, the chain of custody form is formally called the Federal Drug Testing Custody and Control Form, usually shortened to CCF. It is the legal document that follows a urine specimen from the moment it is collected until the final result is reported by the Medical Review Officer (MRO).

Every person who handles the specimen, the collector, the laboratory, the MRO, is documented on it, along with the date, time, and purpose of each transfer.

The form exists to prove one thing: that the specimen tested in the laboratory is the same specimen the donor provided, and that nobody had an opportunity to tamper with it along the way. Without that unbroken record, the result cannot be trusted, and under DOT rules it cannot be used.

Why It Matters for Enterprise Fleets

For a carrier running a handful of trucks, drug testing paperwork is an occasional headache. How the Testing Process Works, Step by Step

The CCF is designed to mirror the physical path of the specimen. Understanding the sequence makes it much easier to spot where things go wrong.

Before Collection: Employer and Test Setup

The employer or its designated third-party administrator supplies the collection site with the information that goes into the top section of the form: the employer's name and address, the MRO's contact information, the donor's identification number (usually a driver's license number or employee ID rather than a Social Security number), and the reason for the test.

Getting this section right upfront matters because a wrong MRO or a wrong reason-for-test code can cause the result to be routed incorrectly or misclassified in your records.

At the Collection Site: The Collector's Role

The collector verifies the donor's identity, has the donor empty their pockets and wash their hands, and provides a sealed collection kit. Once the specimen is produced, the collector must check its temperature within four minutes and record it on the form.

The specimen is then divided into two bottles, a primary and a split, in the donor's presence. Both bottles are sealed with tamper-evident labels that carry the specimen ID number, and the donor initials the seals. The collector then completes the collector certification section, including printed name, signature, date, and time.

Donor Certification

The donor signs a statement certifying that the specimen is theirs and that it was sealed in their presence. This is one of the more commonly missed steps, and while it is generally treated as correctable rather than fatal, it still triggers follow-up and delay.

Shipment to the Laboratory

The specimen bottles and the laboratory copy of the CCF are placed in a sealed shipping container. The chain of custody section on the form records who released the specimen and who received it, at each handoff, until it arrives at the certified lab.

Once at the laboratory, the accessioner checks the seals, matches the specimen ID on the bottle to the ID on the form, and documents receipt.

The Medical Review Officer and the Final Result

The laboratory reports its finding to the MRO, who receives their own copy of the CCF. For a negative, the MRO verifies the paperwork and reports the result to the employer.

For a non-negative, the MRO contacts the donor, conducts an interview to determine whether there is a legitimate medical explanation, and then reports a verified result. That verified result, positive, negative, refusal, or cancelled, is what the employer acts on.

The Five Copies of the Form and Who Keeps What

The paper CCF is a five-part form, and each copy has a designated recipient:

  • Copy 1 goes to the testing laboratory with the specimen

  • Copy 2 goes to the Medical Review Officer

  • Copy 3 stays with the collector

  • Copy 4 goes to the employer

  • Copy 5 goes to the donor

For recruiting and safety teams, Copy 4 is the one that matters most. Under DOT recordkeeping rules, employers must retain drug testing records for defined periods, longer for positives, refusals, and related documentation than for negatives. If your team can't produce the employer copy of a CCF for a driver's pre-employment test during an audit, that gap is a finding regardless of what the result was.

Electronic versions of the form, known as eCCFs, are now widely used and follow the same structure and signature requirements digitally. For high-volume fleets, eCCFs cut down on lost copies and illegible handwriting, though they don't eliminate errors in the underlying data entry.

Common Mistakes That Invalidate or Delay a Test

The Department of Transportation distinguishes between two kinds of CCF errors: fatal flaws that require the test to be cancelled outright, and correctable flaws that can be fixed with a signed memorandum from the responsible party.

Fatal Flaws

These cause the laboratory or MRO to cancel the test, and the specimen cannot be used:

  • The specimen ID number on the bottle does not match the number on the CCF

  • The specimen ID number is missing entirely from the bottle

  • The seal on the primary specimen bottle is broken or shows evidence of tampering (and the split cannot be redesignated)

  • The collector's printed name and signature are both missing from the form

  • Insufficient specimen volume for testing with no valid explanation

Correctable Flaws

These are errors that can be resolved if the collector or other responsible party provides a signed statement correcting the record:

  • The donor's signature is missing without a notation explaining why

  • The collector's printed name or signature is present but not both

  • The certifying scientist's signature is missing on the laboratory copy

  • The employer name or MRO information is incomplete or incorrect

Correctable flaws don't cancel the test, but they do add days to the timeline while the correction is chased down. When your goal is getting a qualified driver from offer to first dispatch as fast as possible, those days matter.

Process Errors That Don't Show Up on the Form

Some of the most damaging mistakes never appear as a flagged field. The temperature check performed late. The specimen split outside the donor's line of sight. Bottles sealed after the donor has already left.

These are procedural failures the collector is supposed to prevent, but they only surface if a result is challenged and the collection is scrutinized. Working with reputable collection sites and periodically auditing collector performance is the only real safeguard.

What Happens When a Test Is Cancelled

A cancelled test is neither positive nor negative. For a pre-employment test, that means the driver cannot be placed into a safety-sensitive function until a new collection is completed with a valid result. The employer must arrange a recollection, and the clock on your onboarding timeline effectively resets for that step.

For random tests, a cancelled result generally means the test doesn't count toward your annual random testing rate, which has its own compliance implications. For post-accident or reasonable suspicion tests, a cancellation can leave a gap in your documentation at exactly the moment you most need a clean record.

The through-line here is that CCF errors don't just create administrative work. They create measurable delays in seating drivers and measurable gaps in your compliance file.

Keeping Chain of Custody Records Organized at Scale

One Record Per Driver, Every Test

The employer copy of every CCF, along with the MRO's verified result, belongs in a secure, retrievable record tied to the driver. At an enterprise fleet, that record needs to be findable by driver, by test type, by date, and increasingly by DOT number or operating company, since auditors reviewing one authority may only need to see that entity's records.

Separating Testing Records From the DQ File

Drug and alcohol testing records are confidential and must be maintained separately from the general driver qualification file, with access limited to authorized personnel. Fleets that store CCF copies loosely inside the same folder as the application and MVR create both a confidentiality problem and an audit finding.

Coordinating With Your TPA and MRO

Most larger fleets work through a third-party administrator for collection scheduling and MRO services. Whoever manages that relationship should have a clear process for receiving verified results, matching them to the correct driver record, and flagging cancelled or non-negative results to recruiting and safety immediately rather than in a weekly report.
